Council Membership:
The application period for the 2011-2012 term ran from March 1, 2011- May 12, 2011 by 5:00 P.M. for New & Returning Members. We are no longer accepting applications to join the council. Please check back in the Spring of 2012 for membership information.

Methods to Submit Your Application:
You can submit your application using any of the below listed methods:
1) Submit by Mail: Complete your application online and print the completed application. You can then mail your application by the May 12, 2011 deadline to the following address:

2) Submit by Fax: Complete your application online and print the application. You can then submit your application by faxing it to the following number:

3) Submit your application electronically via email: Complete the application, then scan and save it to your computer desktop. After the application has been saved on your desktop, you can open an email and include the application as an attachment. Your application should be emailed to the following address:
